Sevita - - Responsibilities: Delivers direct support per individual service plans and develops skills/behavioral development; Assists with daily living activities: personal care, housekeeping, transportation, community integration, and other daily living tasks; Documents progress and reviews records to update service plans; maintains confidentiality; Engages with individuals, families, guardians and case managers; adheres to Sevita customer service standards; Monitors health, accompanies to medical appointments, administers or supports self-administration of medications, and responds to health-related situations as needed
